Along with a Completed Application Form – All parent(s)/guardian(s) of prospective students must provide the following:

Birth Certificate	Immunization Records*
Medication Consent Form	Health Report
Student Media Release Form	Emergency Information
Food Allergies Form	Financial Contract

*Immunization records are not required if a parent or guardian submits a letter from a primary physician explaining that such immunizations are contrary to their beliefs. This letter is to be renewed annually.
